Applying for WrapPlan^®II

If you are interested in applying for WrapPlan^®II, or, if you would like additional information, please contact the Plan Administrator, USI Affinity, toll-free at 866-NYSUT-WP (866-697-8897) to speak with a licensed Benefit Representative.  Representatives are available Monday through Friday from 9:00 am to 9:00 pm and on Saturdays from 10:00 am to 3:00 pm. 

You may apply for WrapPlan^®II via telephone, or you may schedule an appointment to meet with a licensed representative.  For your convenience, representatives are available evenings and weekends in addition to regular business hours.
